At Somerset Family Dentistry, we are always looking for ways to reward our loyal patient base.  Probably the most requested cosmetic procedure our patients enquire about is bleaching their teeth professionally.  Many patients are interested, but due to the cost involved and the fact that dental insurance plans do not reimburse for whitening, many patients are left out.

      But thanks to the Somerset Family Dentistry "Whitening for Life" program, our loyal patients now have a way to get prescription strength teeth whitening for a fraction of the cost.

    To enroll in the program all you have to do is show up to your scheduled recall (cleaning) appointments for one full year, without missing or breaking an appointment (not showing up to an appointment or cancelling without 24 hours notice).  You can not be more than 20 days overdue to your recall appointment.

     All those patients who fulfill the above criteria will be able to get professional at home bleaching trays and prescription strength bleach for $150  (normally- $500). 

     Then as an added bonus, those patients who continue with the terms listed below, will receive a FREE  touchup kit annually ($40 value) for the life of the program:

1. Patient must return to Somerset Family Dentistry for their regularly scheduled cleanings as prescribed by their doctor.
2. If patient comes more than 20 days from their prescribed cleaning the program will be voided.
3. If patient does not give 24 hours notice before cancelling their cleaning, for any reason, or no shows an appointment the program will be voided.
4. If patient loses their trays, or major work is needed that results in the inablity to use their trays, new ones will be fabricated for $150 (disounted rate) if still enrolled in the program.
5. Once you default on any of the terms above you will not be able to re-enroll in the program.  All touchup kits or new trays will have to be purchased at their regular price.
